CHRISTIAN FORMATION ENROLLMENT FORM

Community of St. Malachi and St. Malachi Parish

If you wish to enroll your child (ren) in the Christian Formation Program (open to children age 3 through high school), please take a few minutes to fill out this extra form and return it with your commitment form. Registration fees should accompany this registration form. Fees are due before June 1, 2000. Fees are $25/child, $75/family (maximum). There is a $50 assessment per family for non-members.

The following special programs will be offered to interested families. Please check all the programs in which you will have a child participating this year. The appropriate information will be sent to you.

There are many opportunities to participate in the Christian Formation Program. Parental involvement is strongly encouraged. Please indicate any and all areas in which you would like to be involved.
